Vashon Youth and Family Services (VYFS) is the largest provider of human services on Vashon, serving 15% of the Island population with services including counseling for families, youth and adults, emergency services for the Island’s most vulnerable population, enriched before and after care for elementary school children and family support and education. Summary of responsibilities:  
• Participate in weekly team meetings/consult, weekly supervision and monthly staff meetings 
• Perform duties related to working with clients identified as high risk, to identify needs and goals to achieve empowerment clients and improved quality of life for both behavioral and physical health issues 
• Assess client’s current functional level and, in collaboration with the client, develops and monitors an Individual Service Plan and quality of care 
• Collaborates with both medical and behavioral providers to ensure optimal care for clients 
• Available to consult by telephone with clients for behavioral issues, and their providers to identify needs, set goals and implement action steps towards achieving goals. Empower clients to help them improve their quality of life and ensure an integrated approach to address complex issues 
• Provides ongoing coordination of case management services, assessment, treatment planning, resource identification and development, environmental assistance, advocacy, collateral supports and clinical services as appropriate to assigned caseload 
• Create and support a trauma-informed, safe, trusting healing environment that respects the perspectives and experiences of the individuals, families, staff and communities we serve with safety and healing 
• Hold client confidentiality as a high value, understand and follow all applicable rules of confidentiality, including HIPPA, HITEC and subsequent local and national legislation protecting client privacy, seek additional training and clarification when needed  
• Maintain caseload and paperwork expectations, completing required documentation no later than weekly following client interactions  
• Completes treatment plan within 30 days of intake date 
• Completes At Risk Youth Service plan within 30 days of intake 
• Completes therapist disclosure at first individual session  
• Creates safety plan and referrals to crisis connections when Suicidal ideation, self harm or homicidal ideation is present. Informs supervisor, assistant director, or clinical director within 24 hours when aware 
• Understands duty to warn and makes CPS or APS when warranted as well as informs supervisor within 24 hours of call 

Preferred Qualifications: 
Education, Experience, and Certification: 
 • Experience with child and family strongly preferred 
• Fluent in Spanish in a clinical context, strongly preferred 
• Master’s Degree in related field required by start date, LMHCA, LMHC, LISCW or LMFT 
• Experience working with seriously and persistently mentally ill adult populations 
• 2 years or more experience in community mental health setting 
• Additional certification requirements: Work towards one or more of: Infant mental health specialist, Child mental health specialist, Geriatric mental health, Developmentally delayed mental health specialist 
• Current Washington State Mental Health Professional credential 

Knowledge, Skills and Abilities:  
• Delivers such services with respect, tact and cultural sensitivity.  
• Training and expertise, or a demonstrated willingness to receive training and develop expertise in at least one Evidence-Based Practice modality of intervention appropriate for delivering therapy to children, youth, adults and families.  
• Manages the stress of serving difficult/agitated consumers in a safe and dignified manner as dictated by facility policies and procedures.  
• Works cooperatively in a multidisciplinary team approach, according respect to others based on knowledge and experience as well as discipline.   
• Computer literacy, ability to learn new technology and adapt to change

PAT Home Visitor

Family Place - Vashon, Washington (Hybrid)

POSITION SUMMARY  

The Parents as Teachers (PAT) Home Visitor is a member of the family support team based at Family Place, a program of Vashon Youth and Family services. PAT Home Visitors are trained in the Parents as Teachers foundational curriculum model, an evidence-based program focused on child development, parenting skills, and family well-being. Home visitors plan and deliver bi-monthly home visits with participating families. They provide parent-child play activities that strengthen parenting skills, knowledge of child development and overall family well-being. In addition, home visitors conduct health and developmental screenings and offer support for family empowerment in addressing concerns. This sequence of visits and activities facilitates the growth in relationship between parent(s) and child in order to develop a strong social/emotional foundation, build positive attachment, and support optimum development across the domains of language, cognition, motor skills and social-emotional development.  

The PAT program supports families with children ages prenatal though 5 years old. Participating families are drawn from the broad spectrum of demographics represented on Vashon Island.  

  

QUALITIES AND STRENGTHS REQUIRED FOR SUCCESS IN THIS POSITION:  

We are looking for an individual with strong communication and mature interpersonal skills. These strengths are fundamental to the ability to establish rapport with families and empower them by building on their strengths.  

Success in this position requires strong organizational skills, familiarity with online information management, and the ability to work independently and be self-directed. Demonstrated experience and comfort with a broad spectrum of family configurations, cultures, languages, and life circumstances is necessary. Bilingual capacity in Spanish and English is a plus.  

  

RESPONSIBILITIES:  

  • Provide twice-monthly home visits with each enrolled family, scheduling visits at times that are convenient for the family.  

  • Use the Parents as Teachers curriculum model and resources to structure visits, including child development information; parent-child interaction activities; development-centered parenting, and family well-being discussions and resources.  

  • Prepare each visit using the model planning guide and write timely reports on each visit (critical to fidelity to the PAT program.) Document planning and details of visits, assessments, screenings, referrals, etc. in the PAT Visit Tracker system.  

  • Complete initial and annual developmental screenings and health reviews for each enrolled child; complete initial and annual family-centered assessments with each family, including caregiver well-being screenings.   

  • Connect families to resources that help them address their needs and reach their goals.  

  • Help to plan events to give enrolled families an opportunity to build social connections with each other, engage in parent-child interaction activities, and increase their knowledge of ways to support children's development.  

  • Participate in Family Place team meetings and support the overall mission of Family Place as a resource center for families with young children on Vashon Island.   

  

Core Competencies:  

  • Associate degree or work/life experience sufficient to demonstrate ability to overcome challenges, work successfully across diverse expectations and requirements, stretch beyond your comfort zone when needed and achieve difficult goals.  

  • Experience working with families with young children; demonstrated creativity in providing curriculum and resources.  

  • Knowledge of infant and early childhood development and attachment theory and practice.  

  • Current Washington State driver’s license and auto insurance.  

  • Excellent written and oral communication ability.  

  • Proficiency  with Microsoft Office products including Word, Excel, Outlook, Sharepoint and online collaboration tools
